SMS Consent Form
Tax resolution firms that use TaxResOS collect written consent to text a client before any message is sent. Consent is taken on the firm’s own intake form or engagement letter, at the point the client engages the firm. Reproduced below is the SMS consent block exactly as it appears on that form.

Text message (SMS) consent
By providing my mobile number, I agree to receive text messages about my tax resolution case, sent by my tax resolution firm through its practice-management software TaxResOS. These include case status updates, document requests, appointment reminders, and e-signature requests. Message frequency varies. Message and data rates may apply. I can reply STOP at any time to opt out, or HELP for help. Consent is not required as a condition of receiving services.
Leaving this box unticked does not affect the services the firm provides. A client who consents may stop messages at any time by replying STOP, or by asking the firm to remove the number.
Specimen only — this page collects nothing. Once a client has signed, a staff member records the consent in TaxResOS by entering the mobile number and ticking the per-client “OK to text” box on that client’s case record. TaxResOS blocks every outbound message to a number until that box is ticked, and unticking it stops future messages immediately.
